Our clients are our first priority, if contact us with a crisis situation we are available to review your concerns and make recommendations immediately. We understand the urgency of your concerns and the importance of service and good communication. Care managers can be selected as emergency contacts for clients. If contacted by health care professional or loved ones we can go to the client 24 hours a day 7 days a week. This includes home, hospital, or facility. If in a hospital setting when discharge is being discussed your care manager can obtain information from the hospital discharge planner. This allows your care manager the ability to make recommendation based on personal knowledge of the client, their current condition and upcoming needs.
